At Azalea Orthopedics, our entire team: Orthopedic doctors, Advanced Practice Providers (Nurse Practitioners and Physician Assistants), medical staff, administrative staff and management, work exceptionally hard to deliver the care that our patients expect and deserve. Because East Texas is comprised of many communities across several counties, we pride ourselves in serving patients at conveniently located offices across the region. Patients can be seen by Orthopedic providers who specialize in general Orthopedics, specific body parts, Pain Management, Trauma, Sports Medicine, Spine Care, Physical Medicine and Rehabilitation, and Joint Replacement.

Main Office
Our main office in Tyler is split into three sections, Main Level entrance, Joint Reconstruction Center entrance and the Spine Center entrance.
If you enter our campus from Golden Road, the Main Level entrance (left door) is located adjacent to the Joint Reconstruction Center entrance (right door).
The Spine Center entrance is located on the lower level of the building and is the only entrance that faces Paluxy Drive.
You cannot access the upper levels from the Spine Center or vice versa. All spine and pain management doctors are located in the Spine Center location that faces Paluxy Drive.

Palestine Clinic
Our Palestine clinic is located in the same building as the Baylor Scott & White Texas Spine & Joint Urgent Care Clinic. There is one entrance. Upon arrival, ask for your physician at the front desk.

Nacogdoches Clinic
Our Nacogdoches office is located in North of SFA University on University Dr. This location houses spine, orthopedic surgeons and pain management physicians.

South Broadway Clinic
Our clinic is located in the same building as the Baylor Scott & White Texas Spine and Joint Urgent Care Clinic. Upon arrival, drive to the back of the building and look for the Azalea Orthopedics entrance on the Northside of the building.

Mount Pleasant Clinic
Our Mount Pleasant Clinic is located behind (South East) of the Titus hospital. We are in an independent building behind the hospital.

Athens Clinic
Our Athens location has a new suite number! We’re at the same address, but we’re now in suite number 300 at the back of the building. Providers at this address include: Dr. Bartley, Dr. Burlison, Dr. Harris, Dr. Hester, Dr. Kummerfeld, Dr. Merrick, Dr. Priddy and Dr. Raabe. Advanced Practice Providers include Jordan Cooke, PA-C and Lindsey Markel, FNP-C.

Longview Clinic
Our Longview clinic is located in the Texas Spine and Joint Urgent Care Clinic. There is only one entrance. Upon arrival, ask for your physician at the front desk.
After walking through the Main Level Entrance doors, take a right to find our Occupational Therapy lobby.
