Rater 8 Patient Satisfaction Survey

Increasingly, the Center for Medicare and Medicaid Services (CMS) is moving toward requiring medical groups and acute care hospitals to use these surveys as part of a federal program to reward excellent care and to impose penalties by withholding reimbursement for poor performance. CMS is also moving toward requiring one-third of such surveys to be completed on paper, one-third via phone call, and one-third in the electronic format you are now receiving.

Azalea downloads all information quarterly, so please do not add requests requiring immediate attention in the comments section. Please call your physician’s office for anything that needs personalized and prompt attention.
