Patient Outcome Survey
Our goal at Azalea Orthopedics is to provide you with the best medical care possible. One way we can determine if we are delivering outstanding quality care, is by having you complete surveys. These survey’s or questionnaires, provides the data that your physician needs to more accurately track your progress. You may be asked by your physician to complete one of these surveys.
These surveys collect, “Patient Care Outcomes Data” that gives us information about your quality of life and provides the documentation needed to determine if your pain is improving over time. We are excited to announce that if you are asked to complete a survey, you may do so at the convenience of your own home. These surveys will be mailed directly to the email address you gave us thus it’s important that you provide a correct email address before you leave the doctor’s office or when you call and make an appointment. The emails that contain the surveys may automatically be emailed to you if you qualify to receive them. You may or may not receive one based on your medical condition. Please check your email at least one week before you see your doctor to see if you received the survey. If you are selected to do these surveys, they would need to be completed before every visit so we can monitor your condition.
How do you complete the “Patient Care Outcomes” OBERD questionnaires?
If you have an email address and are asked to complete a survey, it will automatically be sent to you. The survey is created by OBERD and is very easy to use. Upon clicking on the link within the email, you are directed to a simplified login that asks for your birthdate. If you click this link and start the OBERD survey but don’t finish, the link is then expired. You may request a new link that will be sent to your email address. It is critical that you notify the office of your correct email address.
If you do not have an email address and are being asked to complete a survey you can use any computer to log in to the website: https://login.oberd.com/institutions/azalea. You will log in by giving your first and last name, date of birth, and Azalea’s medical records number (you must get this number from your doctor’s medical assistant) or the last four digits of your social security number. Upon logging in, you will see the start page of the questionnaire.
